Vasectomy reversal success rates vary depending on how long ago the original vasectomy was performed. Regular monitoring after surgery helps track your progress and informs decisions about natural conception or assisted fertility treatments. Your first test is typically performed around 12 weeks after surgery, with follow-up tests recommended at intervals to monitor improvement.

Sperm Concentration

Timing: Your surgeon will advise when to have your first test—typically around 12 weeks after your reversal surgery. Follow their guidance on testing intervals.

Abstinence: Avoid ejaculation for 2–5 days before your test. Less than 2 days may give a lower count; more than 7 days may affect motility and increase abnormal forms.

Lifestyle: For the best sample quality, avoid alcohol, recreational drugs, hot baths, saunas, and steam rooms for at least 3 days before your test.

On the day: When you arrive, you'll be shown to a private room. Wash your hands and genitals with soap and water before producing your sample. Do not use lubricants, saliva, or condoms as these can affect results.

Collect everything: It's important to collect the entire ejaculate. The first portion contains the highest concentration of sperm. If any is lost, tell the laboratory staff.

Recent illness: If you've had a fever or infection recently, let the laboratory know. You may be advised to reschedule, as illness can temporarily affect sperm production.

What if no sperm are found on my first test?
Don't be discouraged. It can take several months for sperm production to resume after reversal surgery. Your surgeon may recommend repeating the test in 2–3 months. Some men don't see sperm until 6 months or longer post-surgery.

What's a good result after vasectomy reversal?
The goal is to see sperm present with reasonable motility. Even if counts are lower than WHO normal values, natural conception is often still possible. Your surgeon can advise based on your specific results and your partner's fertility status.

Why might I have anti-sperm antibodies?
After vasectomy, some men develop antibodies against their own sperm. These can persist after reversal and may cause sperm to clump together (agglutination). If antibodies are suspected, additional testing (MAR test) may be recommended.

How many tests will I need?
Most surgeons recommend at least 2–3 tests over the first year to monitor recovery. The frequency depends on your results and fertility goals. If counts are improving, fewer tests may be needed.

What if my results aren't improving?
If sperm counts remain very low or absent after 12 months, your surgeon may discuss options including repeat surgery, sperm retrieval procedures (PESA/MESA), or assisted fertility treatments like IVF/ICSI.

Can I produce my sample at home?
No. For comprehensive semen analysis, the sample must be produced on-site at the laboratory. This ensures the sample is analysed within the optimal time window and at the correct temperature for accurate results.
