Prepare at home before your appointment

Prepare at home

Abstain from ejaculation for 2–5 days before your appointment. Avoid alcohol, saunas, and hot baths for 3 days. Stay hydrated and try to relax — stress can affect your sample quality.

Attend the laboratory in central London

Attend the laboratory

Visit TDL at 76 Wimpole Street, London W1G 9RT. You'll be shown to a private, comfortable room to produce your sample. The laboratory staff are professional and discreet — this is routine for them.

£50 attendance fee: A £50 fee is payable directly to The Doctors Laboratory when you arrive. This covers the use of their facilities and is separate from the test price shown above.

Understand your fertility with a detailed, WHO 2021-standard laboratory assessment

Taking the first step towards understanding your fertility can feel daunting, but knowledge is empowering. A comprehensive semen analysis provides detailed insights into your reproductive health, examining everything from sperm count and movement to shape and overall semen quality—all following the latest World Health Organisation guidelines.

Whether you're planning a family, have been trying to conceive, or simply want to check your fertility status, this test gives you clear, actionable information. Around half of fertility challenges involve male factors, so getting tested early can save valuable time and help you make informed decisions about your next steps.

Your sample is analysed for over 12 parameters according to WHO 2021 guidelines. Here's what we check:

Sperm Concentration

How many sperm are in each millilitre of semen—a key indicator of fertility potential.

Normal: ≥16 million/mL
Total Sperm Count

The total number of sperm in your entire sample, combining concentration with volume.

Normal: ≥39 million
Progressive Motility

The percentage of sperm swimming forward effectively—essential for reaching an egg.

Normal: ≥30%
Total Motility

The overall percentage of sperm that are moving, including those not swimming forward.

Normal: ≥42%
Morphology

The percentage of sperm with normal shape—head, midpiece, and tail all correctly formed.

Normal: ≥4% normal forms
Vitality

The percentage of live sperm, which helps distinguish between dead and live immotile sperm.

Normal: ≥54%
Semen Volume

The total amount of ejaculate. Low volume may suggest collection issues or gland problems.

Normal: ≥1.5 mL
pH Level

Whether semen is appropriately alkaline, which is important for sperm survival.

Normal: ≥7.2
Liquefaction

How quickly semen changes from gel to liquid—normally within 15–30 minutes.

Normal: Complete by 60 min
Viscosity

How thick or thin the semen is after liquefaction. High viscosity can impair sperm movement.

Normal: Flows freely
Appearance

Visual assessment for colour and opacity—can indicate infection or other issues.

Normal: Grey-white, opalescent
Agglutination

Whether sperm are clumping together, which may suggest antibodies affecting fertility.

Normal: None/minimal

Abstinence: Avoid ejaculation for 2–5 days before your test. Less than 2 days may give a lower count; more than 7 days may affect motility and increase abnormal forms.

Lifestyle: For the best sample quality, avoid alcohol, recreational drugs, hot baths, saunas, and steam rooms for at least 3 days before your test.

On the day: When you arrive, you'll be shown to a private room. Wash your hands and genitals with soap and water before producing your sample. Do not use lubricants, saliva, or condoms as these can affect results.

Collect everything: It's important to collect the entire ejaculate. The first portion contains the highest concentration of sperm. If any is lost, tell the laboratory staff.

Recent illness: If you've had a fever or infection in the past 3 months, let the laboratory know. You may be advised to reschedule, as illness can temporarily affect sperm production.

Important Information

This test provides laboratory results only—not a medical diagnosis. Sperm quality can vary naturally from day to day, so a single test showing abnormal values doesn't necessarily mean there's a fertility problem. If results are outside normal ranges, a repeat test 2–4 weeks later is typically recommended before drawing conclusions.

Results are delivered without clinical interpretation. We recommend discussing your results with your GP or a fertility specialist who can put them in context with your overall health and circumstances.

Abstinence: Abstain from ejaculation for 2-5 days before the test (not less than 2, not more than 5).

Sample Collection: Collect by masturbation into the sterile container provided. Do not use condoms, lubricants, or saliva.

Sample Delivery: The sample must reach the laboratory within 1 hour of collection, kept at body temperature. Arrange collection near the laboratory if possible.

Illness: Wait at least 3 months after any febrile illness before testing, as fever can temporarily affect sperm production.
