Prepare at home before your appointment

Prepare at home

Abstain from ejaculation for 2–5 days before your appointment. Avoid alcohol, saunas, and hot baths for 3 days. Stay hydrated and try to relax — stress can affect your sample quality.

Visit TDL at 76 Wimpole Street, London W1G 9RT. You'll be shown to a private, comfortable room to produce your sample. The laboratory staff are professional and discreet — this is routine for them.

£50 attendance fee: A £50 fee is payable directly to The Doctors Laboratory when you arrive. This covers the use of their facilities and is separate from the test price shown above.

Post-Vasectomy Semen Analysis

Confirm your vasectomy was successful with a laboratory clearance test

After a vasectomy, it's essential to confirm the procedure was successful before relying on it as contraception. This post-vasectomy semen analysis checks whether sperm are still present in your semen—the goal is to confirm azoospermia (complete absence of sperm), giving you clearance to stop using other forms of contraception.

This test should be performed at least 12 weeks after your vasectomy and after you've ejaculated at least 20 times since the procedure. This allows time for any remaining sperm to clear from the vas deferens. Until you receive clearance, you must continue using alternative contraception.

⚠️ Important: Do not stop using contraception until you have received a clearance result confirming no sperm are present. Unprotected intercourse before clearance could result in pregnancy.

Understanding Your Results

Azoospermia (No Sperm Detected) This is the result you want. It means no sperm were found in your sample and your vasectomy was successful. You can stop using other contraception.
Non-Motile Sperm Present A small number of non-moving (dead) sperm may still be present. You may be asked to repeat the test in 4 weeks. In some cases, "special clearance" can be given if non-motile sperm persist at very low levels after multiple tests.
Motile Sperm Present If moving sperm are detected, this suggests the vasectomy may not have been successful or has reconnected (recanalization). You should continue using contraception and consult your GP or urologist.

Timing: Wait at least 12 weeks after your vasectomy before testing. This gives time for any residual sperm to clear from your system.

Clear the pipes: You must have ejaculated at least 20 times since your vasectomy before this test. This helps flush out any remaining sperm from the vas deferens above the vasectomy site.

Abstinence: Avoid ejaculation for 2–5 days before your test. No less than 2 days and no more than 7 days.

On the day: When you arrive, you'll be shown to a private room. Wash your hands and genitals with soap and water before producing your sample. Do not use lubricants, saliva, or condoms.

Collect everything: It's important to collect the entire ejaculate. If any of the sample is lost (especially the first portion), please inform the laboratory staff as this may affect the accuracy of results.

Why do I need to wait 12 weeks?
Sperm can survive in the vas deferens above the vasectomy site for several weeks after the procedure. Waiting 12 weeks and having 20+ ejaculations ensures any residual sperm have been cleared, giving an accurate result.

What if sperm are still present?
If non-motile (non-moving) sperm are found, you may be asked to repeat the test in 4 weeks. If motile (moving) sperm are present, this may indicate the vasectomy was unsuccessful or has reconnected—you should consult your GP or urologist.

Will I need more than one test?
Most men only need one test if the result shows azoospermia (no sperm). If sperm are present, you may need follow-up tests at 4-week intervals. Some guidelines recommend two clear samples before confirming success.

Can I produce my sample at home?
No. For post-vasectomy analysis, the sample must be produced on-site at the laboratory. This ensures the sample is analysed promptly and at the correct temperature for accurate detection of any sperm, including whether they are motile.

What happens if my vasectomy failed?
Vasectomy failure is rare (around 1 in 2,000). If motile sperm persist, your urologist may recommend a repeat vasectomy procedure. Continue using contraception until you receive clearance.

Important Information

This test provides laboratory results only—not a medical diagnosis. Results will confirm whether sperm are present in your semen sample and, if so, whether they are motile (moving). A clearance result (azoospermia) means no sperm were detected and your vasectomy was successful.

If your results show sperm are still present, please consult your GP or the surgeon who performed your vasectomy for guidance on next steps. Do not stop using alternative contraception until you have received a clearance result.

Timing: Test at least 12 weeks after vasectomy AND after at least 20 ejaculations post-procedure, whichever is later.

Sample Collection: Collect by masturbation into the sterile container provided.
