The Vitamin B12 (Active) Blood Test measures your active vitamin B12 levels to identify deficiency that could be affecting your energy, mood, and overall health. Vitamin B12 is essential for the production of red blood cells which carry oxygen around the body, and it plays a crucial role in metabolism and the nervous system. Prolonged lack of vitamin B12 can cause nerve damage, anaemia, and a range of symptoms including extreme tiredness, muscle weakness, and poor memory.

This test is ideal for those experiencing symptoms of low vitamin B12 such as extreme tiredness, low energy, muscle weakness, or poor memory. It is particularly useful if your diet lacks B12-rich foods like meat, fish, or dairy, or if you have a health condition that affects absorption such as Crohn's disease, coeliac disease, or pernicious anaemia. Vegetarians and vegans are also at higher risk of B12 deficiency, although many plant-based products are now fortified with vitamin B12.

Vitamin B12 Supplements: Do not take vitamin B12 supplements for 2 weeks before this test, as they can artificially elevate your results and mask a true deficiency. If your B12 is prescribed by your doctor, please consult with them before stopping your medication. Biotin Supplements: Do not take biotin (vitamin B7) supplements for at least 48 hours before this test, as biotin can interfere with the laboratory assay and produce inaccurate results. If biotin has been prescribed by your doctor, discuss the timing with them. Other Supplements: Take your sample at least 24 hours after any other vitamin or mineral supplements to ensure accurate baseline measurements. Fasting: No fasting is required for this test. Test Limitations: If your active B12 result is greater than 150 pmol/L, it will be reported as >150 pmol/L. Results at the upper limit are clinically reassuring and indicate adequate B12 status.

Active vitamin B12, also known as holotranscobalamin, is the biologically available form of vitamin B12 that your cells can actually use. Unlike total B12 tests (which measure both active and inactive forms), active B12 provides a more accurate picture of your true vitamin B12 status. Vitamin B12 is essential for producing red blood cells which carry oxygen around the body, and it plays a vital role in metabolism and the nervous system. Prolonged deficiency can cause nerve damage, pernicious anaemia, extreme tiredness, muscle weakness, poor memory, numbness or tingling in the hands and feet, and mood changes. Vitamin B12 is found almost entirely in animal-based foods such as meat, fish, eggs, and dairy, though many vegetarian and vegan products are now fortified with B12. Deficiency is more common in vegetarians, vegans, older adults, and those with conditions affecting absorption such as Crohn's disease, coeliac disease, or atrophic gastritis. Low levels should be discussed with your GP, who may recommend further testing or supplementation.
