The Varicella Zoster Antibodies (IgM) Blood Test detects recent or current infection with varicella zoster virus (VZV), which causes both chickenpox (varicella) and shingles (herpes zoster). IgM antibodies are produced early in the immune response and indicate active or very recent infection.

This test is appropriate for individuals with symptoms suggestive of chickenpox or shingles, those who have been in contact with an infected person and are uncertain of their immunity status, and pregnant women who may have been exposed to VZV. Identifying acute VZV infection is particularly important during pregnancy.

No Fasting Required: This test does not require fasting. You may eat and drink normally before your blood collection. Timing After Symptoms: VZV IgM antibodies typically become detectable within the first week of symptom onset. Testing very early in the illness may occasionally produce a false-negative result. Chickenpox vs Shingles: This test detects IgM antibodies to VZV, which can be positive in both primary infection (chickenpox) and reactivation (shingles). Clinical presentation helps distinguish between these conditions. Pregnancy Alert: If you are pregnant and have been exposed to chickenpox or suspect you have VZV infection, please contact your midwife or GP urgently. VZV infection during pregnancy can cause complications and may require treatment with antiviral medication or varicella zoster immunoglobulin (VZIG). Difference from IgG Testing: This IgM test detects recent or current infection. If you need to check your long-term immunity status (for occupational health or pre-pregnancy screening), you should request the Varicella Zoster IgG test instead.

Varicella zoster virus (VZV) IgM antibodies are produced early in the immune response to VZV infection. They typically appear within the first week of symptom onset and may remain detectable for several weeks to months. Their presence indicates current or recent VZV infection. What Positive Means: A positive VZV IgM result indicates current or recent infection with varicella zoster virus. This may represent primary infection (chickenpox) or viral reactivation (shingles). In pregnant women, a positive result requires urgent medical assessment as VZV can cause fetal abnormalities, particularly in early pregnancy, and neonatal varicella if infection occurs around the time of delivery. What Negative Means: A negative VZV IgM result suggests no current or recent VZV infection. However, if tested very early after exposure or symptom onset, antibodies may not yet have developed. A negative result does not indicate immunity—for immunity assessment, an IgG test is required. Chickenpox (Varicella): Primary VZV infection causes chickenpox, characterised by an itchy, blistering rash that progresses through stages (macules, papules, vesicles, crusts), fever, and general malaise. It is highly contagious and spreads through respiratory droplets and direct contact with lesions. Shingles (Herpes Zoster): After primary infection, VZV remains dormant in nerve ganglia and can reactivate later in life to cause shingles. This presents as a painful, blistering rash typically affecting a single dermatome (band of skin supplied by one nerve). Shingles can occur at any age but is more common in older adults and immunocompromised individuals.
