The Tetanus Antibody Blood Test measures your protective antibody levels against tetanus, a serious bacterial infection that affects the nervous system and causes painful muscle contractions. This test determines whether you have adequate immunity from vaccination or whether a booster dose may be needed.

This test is valuable for individuals uncertain about their vaccination history, those who have sustained a potentially contaminated wound and need to determine if a booster is required, travellers to areas with limited medical facilities, or anyone wanting to confirm their tetanus protection status.

No Fasting Required: This test does not require fasting. You may eat and drink normally before your blood collection. Recent Vaccination: If you have recently received a tetanus-containing vaccine, wait at least 4 weeks before testing to allow sufficient time for antibodies to develop and reach peak levels. Wound Management: This test is useful for assessing baseline immunity but should not delay wound treatment. If you have sustained a potentially contaminated wound (especially deep or dirty wounds), seek immediate medical attention. Healthcare providers can administer a tetanus booster and/or tetanus immunoglobulin based on the wound type and your vaccination history. UK Vaccination Schedule: In the UK, tetanus vaccination is given as part of the childhood immunisation programme (at 8, 12, and 16 weeks, and boosters at 3 years 4 months and 14 years). Adults who completed the UK schedule are considered protected for life, though boosters may be needed after certain injuries.

Tetanus antitoxin antibodies are produced by the immune system in response to tetanus vaccination. These antibodies neutralise the tetanus toxin produced by Clostridium tetani bacteria, preventing the severe muscle spasms and potentially fatal complications of tetanus infection. Interpreting Levels: Antibody levels are typically reported in IU/mL. Levels ≥0.1 IU/mL are generally considered protective. Levels between 0.01–0.1 IU/mL indicate some protection but a booster may be recommended. Levels below 0.01 IU/mL indicate inadequate protection and vaccination is recommended. What Protective Levels Mean: If your antibody level is protective (≥0.1 IU/mL), you have adequate immunity against tetanus. However, immunity can wane over time, so periodic reassessment may be helpful, particularly if you are at increased risk of exposure (e.g., outdoor workers, gardeners, travellers to remote areas). What Low Levels Mean: If your antibody level is low or undetectable, you may not be adequately protected against tetanus. A primary course or booster vaccination is recommended. Your GP or a travel clinic can advise on the appropriate vaccination schedule. About Tetanus: Tetanus is caused by bacteria found in soil, dust, and animal faeces. The bacteria enter the body through wounds, particularly deep puncture wounds, burns, or wounds contaminated with soil. Tetanus causes severe muscle stiffness and spasms ("lockjaw") and can be fatal. Unlike many infections, tetanus is not spread person-to-person, and immunity does not develop naturally from infection—vaccination is the only way to build protection.

Frequently asked questions

Tetanus immunity from vaccination wanes over time. The NHS recommends 5 doses through childhood/adolescence for lifelong protection. Checking antibody levels confirms whether you need a booster.

Anti-tetanus IgG above 0.1 IU/mL is generally protective. Below this, discuss a booster with your GP.
