Lipoprotein-associated phospholipase A2 (Lp-PLA2), also known as the PLAC test, measures an enzyme that plays a key role in arterial inflammation and the development of atherosclerotic plaque. Unlike standard cholesterol tests, Lp-PLA2 is produced directly within arterial plaques, making it a specific marker for vascular inflammation and unstable plaque—the type most likely to rupture and cause heart attacks or strokes.

This test is ideal for individuals concerned about cardiovascular health, those with a family history of heart disease or stroke, people with borderline or elevated cholesterol who want deeper insight into their risk, and anyone who has already had a cardiovascular event and wants to monitor their ongoing risk. The PLAC test provides information beyond standard lipid panels, helping to identify "hidden" cardiovascular risk in people whose cholesterol levels appear normal.

Venous Test Kit

This test requires a venous blood sample collected by a trained phlebotomist—it cannot be done with a finger-prick at home. You'll receive a collection kit containing everything the phlebotomist needs.

  • Make Your Own Arrangements: You're welcome to arrange collection with any qualified phlebotomist or clinic of your choice.

Please Note: The phlebotomy service (blood draw) is not included in the test price. Clinics typically charge £20–£50 for this service. Please confirm the fee when booking your appointment.

Fasting: No fasting is required for this test. You can eat and drink normally before your blood draw.

Timing: This test can be taken at any time of day. Lp-PLA2 levels are relatively stable and not significantly affected by recent meals.

Medications: Continue taking your regular medications as prescribed, including statins or other cholesterol-lowering medications. Inform your healthcare provider about all medications you're taking when discussing your results, as some treatments may affect Lp-PLA2 levels.

Recent Illness: If you've recently been unwell with an infection or other acute illness, consider waiting 2-3 weeks after recovery before testing, as acute inflammation can temporarily affect results.

Test Limitations: This test does not diagnose heart disease but identifies vascular inflammation that may contribute to cardiovascular risk. Results should be interpreted alongside other cardiovascular markers including lipid panel, hs-CRP, and clinical risk factors. A single elevated result does not confirm disease—discuss your results with your healthcare provider.

Lipoprotein-associated phospholipase A2 (Lp-PLA2) is an enzyme produced by inflammatory cells within atherosclerotic plaques in artery walls. It plays a direct role in vascular inflammation by generating pro-inflammatory and cytotoxic products that destabilise plaques, making them more likely to rupture. This makes Lp-PLA2 a specific marker for vascular inflammation and vulnerable plaque.

Unlike hs-CRP, which reflects general systemic inflammation from any source, Lp-PLA2 is produced predominantly within arterial plaques themselves, making it more specific to cardiovascular risk. Studies have shown that elevated Lp-PLA2 is associated with approximately doubled risk of coronary heart disease and stroke, independent of traditional risk factors including LDL cholesterol. The PLAC test is FDA-cleared as an aid in assessing risk of coronary heart disease and ischaemic stroke.

Elevated Lp-PLA2 levels (generally above 200 ng/mL or 235 nmol/min/mL depending on assay method) indicate increased vascular inflammation and may warrant more aggressive cardiovascular risk management. Lifestyle modifications (diet, exercise, smoking cessation) and certain medications (particularly statins) can lower Lp-PLA2 levels over time. Results should be discussed with your GP or cardiologist, who can interpret them in the context of your overall cardiovascular risk profile.
